Dr Nick’s View: When a Same-Day Crown Is Not the Right Choice

But as a family dentist, I also believe that the best dentistry is not defined by speed alone. A same-day crown should be chosen because it is the right clinical option for the tooth—not simply because it is convenient.
When is a same-day crown not suitable?
A same-day crown may not be suitable when a tooth needs more observation, complex cosmetic design, further gum management or a staged treatment plan. The right choice depends on the remaining tooth structure, your bite, surrounding gums and the goals for the final restoration.
My Rule: Predictability Before Speed
When I assess a crown case, I begin with a simple question: what gives this tooth the best chance of functioning comfortably over time? The answer can be same-day treatment, but it can also be a carefully staged approach. Modern digital technology is incredibly valuable, yet it does not replace clinical judgement.
Three Situations That Need Extra Thought
1. Complex bite forces
Every crown has to work in harmony with the way you close, chew and move your jaw. If a tooth is subject to unusually high bite forces or if there are signs of clenching and grinding, I may recommend more detailed planning before the final restoration is made. This supports a crown that is designed for your bite rather than simply made quickly.
2. High-aesthetic front teeth
Front teeth can demand exceptional attention to translucency, texture, shade and the way light interacts with the restoration. Our in-house laboratory helps enormously because our dental and lab teams can work directly together. However, some complex smile cases still benefit from a staged process, particularly where we are designing multiple teeth or managing delicate gum contours.
3. Teeth with uncertain long-term prognosis
Occasionally, a tooth has a fracture, infection history or remaining structure that requires additional treatment or monitoring before it is ready for its final crown. In those cases, placing a permanent restoration immediately may not be the most conservative choice. Taking the right steps in the right order is part of protecting the patient from avoidable re-treatment.
Does choosing a traditional crown mean lower quality?
No. A traditional crown is not an inferior crown; it is a different workflow. When a staged approach is clinically appropriate, it can create the time needed to refine fit, function and aesthetics. The quality comes from careful diagnosis, material selection, laboratory collaboration and final adjustment.
